Back-to-School Basics

As we approach the start of the 2017-18 school year, St. Luke's would like to provide families with all the information needed to start the year off right! 

STUDENT INFORMATION FORMS

The Student Information Forms are used to collect information about emergency contacts, medical conditions and allergies, authorized individuals for student pick-up, and more. The forms are pre-populated with information we have on file from the RenWeb database. Please ensure that all information is current and correct. The Student Information Forms can be accessed via ParentsWeb on the lefthand navigation under the School Information tab and Web Forms. If you are experiencing trouble accessing ParentsWeb, please call the school office at (210) 826-0664.

PERMISSION SLIPS

There are additional paper permission slips that must be completed before the beginning of the school year. Please print the packet that matches your child(ren)'s grade level for the upcoming school year. These permission slip packets may be mailed, emailed, faxed, or delivered to the school by August 1st. 

ALL STUDENT INFORMATION FORMS & PERMISSION SLIPS MUST BE COMPLETED BY AUGUST 1ST.

ADDITIONAL SCHOOL FORMS NEEDED

  • Hearing & Vision: St. Luke's must have a current Hearing and Vision form on file for all new students, 4-year olds, 5 year-olds, 1st, 3rd, 5th, and 7th grade students. (Click here for Hearing & Vision Form)
  • Immunization Records: St. Luke's must have your child's up-to-date immunization record on file. Immunization records are required for all grade levels. These records must be turned into the office prior to the first day of school.
  • Sports Physical Examination: If your child is in grades 5-8 and intends to participate in athletics, St. Luke's must have a physical on file. (Click here for the Physical Examination Form)
  • Medication: Complete the Medication Form to authorize St. Luke's to dispense or administer prescribed and over-the-counter medication as directed. (Click here for the Prescription Medication Form). All medication must be brought to school in the original container and will be administered according to label directions. SLES does not provide any medication. 
MEDICAL FORMS MUST BE TURNED IN TO THE SCHOOL OFFICE BY MONDAY, AUGUST 1ST.

SCHOOL UNIFORMS

St. Luke's students are required to dress in the proper uniform for their academic grade level as outlined in the Parent-Student Handbook.

  • Parker School Uniforms supplies the uniforms for St. Luke’s. Please refer to the Parker Uniform Brochure or the SLES Graphic Uniform Guide.
  • School Shoes Unlimited provides the uniform shoes for St. Luke’s. The store stocks a wide variety of styles and sizes of shoes that are acceptable to wear according to the St. Luke's uniform guidelines. Refer to the SLES School Shoes Unlimited brochure. If you are not purchasing your shoes at School Shoes Unlimited and have a question about a particular brand, please feel free to check with the office. 

SCHOOL SUPPLIES

  • Early Childhood, Elementary, and Intermediate level students have school supplies provided for them.
  • Middle School students are responsible for purchasing the supplies on our Middle School Supply List.
  • For safety reasons, rolling backpacks are not allowed at St. Luke’s. Please avoid the use of large bags and purchase a lightweight backpack that can fold in half for storage.
